چکیده

Introduction Mismatch of patients and prostheses is a critical issue for patients undergoing aortic valve replacement (AVR), because it increases postoperative mortality. Mismatch can be determined by measuring the effective orifice area (EOA) of a prosthetic valve. Although EOA is usually measured by transthoracic ultrasonography, its accuracy remains unclear because the prosthetic valve and/or turbulent flow may produce artifacts and results depend on the examiner’s skill.

Impact of patient-prosthesis mismatch and aortic valve design on coronary flow reserve after aortic valve replacement.

OBJECTIVES This prospective-randomized study investigated the effect of aortic valve design and patient-prosthesis mismatch (PPM) on coronary flow reserve (CFR) after mechanical or biological aortic valve replacement (AVR) in patients with aortic stenosis (AS). BACKGROUND Coronary flow reserve may be an important parameter of long-term survival after AVR in patients with AS. Reduced CFR may c...

Excellent durability of Starr-Edwards ball valves implanted in the aortic and mitral positions for 27 years: report of a rare surgical case.

A 59-year-old male who had undergone aortic and mitral valve replacement with Starr-Edwards ball valves 27 years ago was admitted to our hospital for hemolytic anemia and heart failure. Echocardiography revealed prosthetic valve failure with a high-pressure gradient and small effective orifice area. The Starr-Edwards ball valves were successfully replaced with bileaflet mechanical valves. The e...

The Measurement of Opening Angle and Orifice Area of a Bileaflet Mechanical Valve Using Multidetector Computed Tomography

BACKGROUND AND OBJECTIVES The aim of this study was to assess mechanical valve function using 64-slice multidetector computed tomography (MDCT). SUBJECTS AND METHODS In 20 patients (mean age, 50+/-12 years; male-to-female ratio, 10:10), 30 St. Jude bileaflet mechanical valves (15 aortic and 15 mitral valves) were evaluated using MDCT. We selected images vertical and parallel to the mechanical...

Star GK Bileaflet Mechanical Valve Prosthesis-Patient Mismatch After Mitral Valve Replacement: A Chinese Multicenter Clinical Study

BACKGROUND The aim of this study was to investigate the incidence and immediate and mid-term effects of heart valve prosthesis-patient mismatch (PPM) after mitral valve replacement using the GK bileaflet mechanical valve. MATERIAL AND METHODS A total of 493 cases of mechanical mitral valve replacement were performed in the departments of cardiac surgery in 7 hospitals from January 2000 to Jan...

4D flow jet shear layer detection method for the measurement of effective orifice area and assessment of aortic stenosis severity

Background Aortic stenosis (AS) is the most common cause of valve replacement and its severity is mainly assessed by transthoracic Doppler echocardiography (TTE) to quantify valve effective orifice area (EOA) as determined by the continuity equation. In a previous study we demonstrated that EOA can be directly determined with 2D flow MRI downstream of the AS using the jet shear layer detection ...
